During 1860 in London Florence nightingale proposed to international statistical Congress the first model of systematic collection of hospital data. In 1893 it is known as an international list - cause of death was introduced by Jacques Bertillon at the international statistical institute in Chicago. During 1898 American public health association (APHA) recommended revising the system every 10 years to ensure that that it remains updated with current medical practices. The book was small containing both alphabetic index and tabular list. Letter on 6th revision the classification of the book systems expanded in two volumes included morbidity and mortality conditions and it was titled as international statistical classification of disease injuries and cause of death ICD. After 1948 WHO took the responsibility to revise and published the book every 10 years. ICD is most widely used statistical classification book for a disease that been used by countries such as Australia, Canada, and the United States they developed their own adaptations of ICD with more procedure codes for classification for operative and diagnostic procedure. There are many versions of ICD such as ICD 6, ICD 7, ICD 8A, ICD 9, ICD 9 CM, ICD 10, ICD 10CM, ICPM, ICD 10 CA, ICD 11.


ICD 10 CM:-


ICD 10 CM(international classification of diseases 10th revision clinical modification)is based on ICD 10 which replaces ICD 9. It consists of three characteristics of a given code all the way up to 7 characters in some situations. It consists of 21 chapters and contains some new convention that guides code assignment.


CMS (CENTRE FOR MEDICARE AND MEDICAID SERVICES) and NCHS(NATIONAL CENTRE FOR HEALTH STATISTICS) these two department under DHHS(DEPARTMENT OF HEALTH AND HUMAN SERVICES) provide the guideline for ICD 10CM. This is published on the NCHS website. ICD 10CM guidelines have been approved by four organizations are AHA(American hospital association) AHIMA(American health information management association), CMS and NCHS.


The guideline is based on coding sequencing instruction in the tabular list and alphabetic index. This guideline is adhered to and adopted under HIPPA regulation.

There are four sections in this book:-

Section 1 includes the structure and convention of the classification, general, and specific guidelines.

Section 2 to include garland to select principal diagnosis for non-outpatient.

Section 3 e guideline for reporting additional diagnosis in non-outpatient

Section 4 for outpatient coding and reporting.
